Complete the short application and agreement below, and our team will verify your business information and contact you to finalize your launch. There is no required employer cost, setup fee, or minimum employee participation.
We keep the process easy while still protecting Corporate Partner pricing for legitimate participating businesses.

This agreement establishes a voluntary Corporate Wellness Partnership between your business and Ideal Protocol Medical Weight Management.
The Company and Ideal Protocol Medical Weight Management are establishing a voluntary Corporate Wellness Partnership to make Ideal Protocol weight-management programs and Corporate Partner pricing available to eligible Company employees and eligible family members.
There is no required setup fee, employer contribution, or minimum employee participation. Participants enroll with and pay Ideal Protocol directly.
Ideal Protocol will provide approved Corporate Partners with employee benefit materials, preferred Corporate Partner pricing, consultation and enrollment support, local Roseville and Folsom care, and virtual care where available. Medical eligibility, treatment recommendations, prescribing decisions and clinical services remain between the individual participant and the appropriate medical providers.
Corporate Partner pricing is intended for eligible Company employees and eligible family members. Participants may be asked to identify their relationship to the Company and confirm eligibility when requesting the benefit.
Employee participation and medical information are handled directly by Ideal Protocol and its clinical partners. Ideal Protocol does not provide individual medical information to the Company without the participant’s authorization. The Company is not responsible for collecting or managing employee medical information for this program.
The Company agrees to identify an authorized primary contact and reasonably make approved program information available to eligible employees. Participation may include distributing approved email announcements, flyers, landing-page links, employee-benefit materials or allowing an optional wellness presentation.
Each party authorizes the other to use its business name and logo solely for mutually approved materials related to this Corporate Wellness Partnership. Either party may discontinue future use upon termination of the partnership.
Current Corporate Partner pricing will be communicated through Ideal Protocol’s approved pricing materials. Corporate Partner pricing becomes available only after the partnership has been verified and activated by Ideal Protocol. Future pricing may change with notice and will not alter programs already purchased by participants.
The partnership begins when Ideal Protocol confirms activation and continues until either party ends the partnership by written notice. Participants who have already purchased a program may complete the period they purchased.
This partnership is non-exclusive and does not require the Company to purchase services, guarantee participation or recommend any particular medical treatment.
Corporate Wellness Partnerships are intended for legitimate operating businesses with an active workforce. Submission of this application does not automatically activate Corporate Partner pricing. Ideal Protocol may verify business information and approve Corporate Wellness Partnerships at its discretion.
By submitting the application below, the authorized Company representative certifies that the information provided is accurate, that the Company is a legitimate operating business, and that Ideal Protocol may reasonably verify the business information provided before activating the Corporate Wellness Partnership.
